Part 1 — Documents to gather *before* you apply
**Social Security number** and proof of age (birth certificate)
**Work history** — names, addresses, and dates for every job in the last 15 years (SSDI work-credit requirement)
**Medical records** — list of every doctor, hospital, clinic, and therapist, with addresses, phone numbers, and dates of treatment
**Medication list** — every prescription with dosage, prescribing doctor, and side effects you experience
**Test results** — labs, imaging, biopsies, sleep studies, tilt-table results, genetic testing, etc.
**Symptom diary** — dates, triggers, severity, and how symptoms limit daily activities (especially important for invisible or episodic conditions)
**Statements from people who know you** — family, friends, or former coworkers describing what they observe day to day
**School records** (if applicable) — IEPs, 504 plans, accommodation letters
**Bank/financial records** — for SSI, proof of income, resources, and living arrangements
Part 2 — Filing the application
Apply online at [ssa.gov/applyfordisability](https://www.ssa.gov/applyfordisability), by phone at **1-800-772-1213**, or in person at a local Social Security office
Complete the **Adult Disability Report** thoroughly — describe *functional limitations* (what you cannot do), not just diagnoses
Sign medical **release forms** so SSA can request your records directly
Save/write down your **confirmation/receipt number** and the name of anyone you speak with
Note your **alleged onset date** (when you became unable to work) — be consistent everywhere
Part 3 — After you apply
Watch for mail from SSA and from **Disability Determination Services (DDS)** — respond to every request promptly
Attend any scheduled **consultative exam (CE)** — missing it can lead to denial
Keep treating — **gaps in medical care** hurt your case; keep every appointment you can
Continue your **symptom diary** while the claim is pending (decisions often take months)
Do **not** post about activities on social media that contradict your limitations
Part 4 — If you are denied (most initial claims are)
**File a Request for Reconsideration** within **60 days** of the denial letter — do not miss this deadline
If reconsideration is denied, request a **hearing before an Administrative Law Judge (ALJ)** within 60 days
Consider hiring a **disability attorney** — they typically charge nothing upfront and take a capped percentage of back pay only if you win
Submit **new medical evidence** at every appeal stage
Key reminders for invisible & episodic conditions
Describe your worst days , and how often they occur — not just your best days. Explain fluctuation : "I can sometimes do X, but afterwards I crash for days" is powerful evidence. Third-party statements (family, friends, coworkers) help document what medical records alone may miss. Keep copies of everything you submit.
